  • anatomical: sphincter dysfunction, hiatus hernia
  • physiological: smoking, alcohol, spicy/fatty foods, large meals at night, pregnancy, obesity, drugs (e.g. anticholinergics)

  • lifestyle changes
  • antacids, full dose PPI, or H2 receptor antagonist if PPI contraindicated
  • surgery: hernia repair, junction repair
