De Quervains tenosynovitis
swelling + inflammtion of tendon sheaths in wrist
repitive strain injury - pain on radial side of wrist
which tendons are primarily affected in De Quervains tenosynovitis
Abductor pollics longus
extensor pollicis brevis
De Quervains tenosynovitis presentation
sx at radial aspect of wrist near the base of thumb
abduction aggaint resistance is painful
- pain - can radiate to forearm
- aching
- burning
- weakness
which test is specific for De Quervains tenosynovitis
Finkelsteins
- fist with thumb inside
- wrist adducted/ulnar deviated
if pain on radial aspect = De Quervains tenosynovitis
De Quervains tenosynovitis management
analgesia
steroid injection
immobilisation with a thumb splint (spica) may be effective
surgical treatment is sometimes required
Dupuytren’s contracture causes
manual labour
phenytoin tx
alcoholic liver disease
diabetes mellitus
trauma to hand
Dupuytren’s contracture features
ring + little finger
60-70% have fam hx
when should surgical management be considered in Dupuytren’s contracture
when the metacarpophalangeal joints cannot be straightened and thus the hand cannot be placed flat on the table
- -> fasciectomy
- cx = neurovascular damage
commonly recur
