DDx for Globus Pharyngus disorder?

A

Globus pharyngeus is a sensation of a lump or foreign object in the throat, even when there is no physical obstruction. It often occurs due to heightened sensitivity of the muscles and nerves in the throat and is typically not a result of a structural issue

  • Retrosternal Goiter
  • CA laryngopharynx
  • Esophageal or cricothyroid web
  • Skeletal muscle disorders
  • MG
  • Myotonia dystrophica
  • Polymyositis

A

- Thyroid disease - TSH
- Renal artery stenosis - Renal artery Doppler USS
- Aortic coarctation - Doppler echo
- Cushing’s disease - Late-night salivary cortisol, 24hrs urine free cortisol
- Kidney disease - eGFR, Urine ACR
- Pheochromocytoma - Urinary / Plasma metanephrines and catecholamines
- Hyperaldosteronism - Aldosterone/ Renin ratio
- Drugs
- Sleep apnea - Sleep studies or Polysomnography

A

COSTVMPF
- Contraception
- Obesity
- Surgery
- Travel history: Did you walk and stretch your legs?
- Varicose veins: Any dilated blood vessels in your legs?
- Malignancy: Have you experienced any weight loss, loss of appetite, lumps, or bumps?
- Pregnancy: What was the date of your last menstrual period (LMP)?
- Family history of clotting disorders?

DDx for a man coming with Back pain for 2 days?

A
  • Fracture: Any trauma ? Fall down?
  • Osteoporosis : menopause ? HRT? Steroid ?
  • Osteoarthritis: any other joint pain? Morning stiffness?
  • Ankylosing spondylitis: morning stiffness? Eye problem? Postural problems
  • Pagets disease: deafness? ↑ Hat size/ shoe size?
  • Discopathy: lifting heavy object
  • Malignancy: wt loss ? LAP?
  • Fibromyalgia: pain in all of the body
  • PMR: shoulder & hip pain
  • Sciatica: shooting pain
  • Spinal stenosis : pain on walking and problems with bowel and bladder
  • Infection: fever ? Redness?
