Head Injury:

Nice guidance on investigation

CT Head Immediately

A
  • GCS< 13 on initial assessment
  • GCS< 15 at 2 hours post injury
  • suspected open or depressed skull fracture
  • Any sign of basal skull fracture (haemotypanum (ear), panda eyes, cerebrospinal fluid leakage from ear to nose, battle sign (fracture of middle cranial fossa and extravasion of middleauricular artery)
  • Post traumatic seizure
  • Focal neurological deficit
  • > 1 episode of vomittng

CT Head Within 8 hours

A

For adults with any of the folllowing risk factors who have experienced some loss of conciousness or amnesia since the injury:

  • age 65 or older
  • any history of bleeding or clotting disorders
  • dangerous mechanism of injury
  • more than 30 mins of retrograde amnesia of eents immediately before the head injury

A

If a patient is on warfarin who has sustaineda head injury with no otherindications for a CT, perform a CT head scan within 8 hours of the injury
