what is heart failure?
heart failure is the inability of the heart to maintain adequate cardiac output to meet the metabolic needs of the body because of impaired pumping ability
what is the primary cause of heart failure?
hypertension
coronary artery diease
myocardial infarction
pulmonary hypertension
there are two types of heart failure
left vs right sided
what does each mean ?
left side is when the heart is unable to pump blood snd oxygen over to the lungs and the rest of the body
right is when the the blood and oxygen is unable to pump to the lungs and retracts back into the body
clinical manifestation of left sided heart failure ?
pulmonary edema
dyspnea
tachypnea
crackles in lungs
paroxysmal nocturnal dyspnea
increased blood pressure
clinical manifestations of right sided heart failure?
dependent edema
jugular venous distention
abdominal distention
hepatomegaly
splenomegaly
anorexia
weight gain
swelling
what is the medication mainly used for heart failure ?
digoxin
- remember given over 60 hr and toxicity is halo lights - treatment with digifab
what other medications other than digoxin do we give to these patients with heart failure?
diuretics
ace inhibitors
low dose beta blockers
vasodilator
what is the diagnostic tool that we used to be able to confirm heart failure ?
BNP > 100
weight gain of 3 pounds over 2 days
or
5 pounds over a week
should be reported to the healthcare provider
pulmonary edema is the first sign of acute decompensated heart failure
what is the gold standard therapy for patients with heart failure? typically end stage
heart transplant
