Conduction is the transfer of heat energy without the physical movement of medium
Which state of matter conducts heat the best. Why?
Solids are better conductor of heat than fluids (water ad air). As the particles between solids are more closer thus the particles will gain heat quickly. Not only that fluids are a poor conductor of heat.

Radiation is the transfer of heat from a hotter body to a cooler body without a medium

ONLY fluids convects heat. This is because particle have to rise and fall for convection to occur and in solid they are closely packed thus they cannot move for convection to occur.
Where does radiation occur?
It occurs both in a medium and non-medium
