  • Assess for signs of bleeding and hemorrhage.
  • Assess patient for evidence of additional or increased thrombosis.
  • Observe injection sites for hematomas, or inflammation.
  • Monitor platelet count every 2–3 days throughout therapy.
  • Have 2nd nurse check

  • report any symptoms of unusual bleeding or bruising to health care provider immediately.
  • not to take medications containing aspirin or NSAIDs while on heparin therapy.
  • use a soft toothbrush and electric razor during heparin therapy.
