What does a positive trendelenburg test show?
The pelvis will dip on the unsupported side in a positive trendelenburg test
This is due to failure or weakness of the hip abductors on the opposite side!)
At the end of every MSK examination what should you say?
I would like to examine the neuro vascular status of the joint/limb and examine the joint above and below
How would you treat an extra capsular neck of femur fracture?
Dynamic hip screws
How would you treat an subtrochanteric femur fracture?
Intramedullary nail
How would you treat a young patient with an undisplaced intracapsular neck of femur fracture?
Open reduction, internal fixation and Cannulated hip screws (dynamic hip screw)
How are intracapsular NOF fractures classified?
Using the Garden classification
1,2 give a screw, 3,4 do some more
How would you treat an intracapsular NOF fracture Garden classification 1 or 2?
Open reduction, internal fixation with screws
How would you treat an intracapsular NOF fracture Garden classification 3 or 4?
If <55yrs: open reduction, internal fixation with screws (follow up in OPD and do arthroplasty if AVN develops)
If 55-75: total hip replacement
If >75: hemiarthroplasty (cemented or uncemented)
