Where does the femoral head get its blood supply from
Mainly from medial circumflex artery (terminating branches called lateral epiphysial artery), secondly from the lateral circumflex artery. Both come from deep femoral artery
Small part from inferior gluteal artery
What are the regions of the NOF
Intracapsular - from subcapital to intertrochentaric line
Extra capsular - imtertrochemtaric reigon and subtrochentaric region(from lesser trochanter to 5cm distal)
Where does the joint capsule of the NOF end
Inter trochentaric line
Classification of intracapsular fractures
Gardens
1- non complete and non displaced
2- complete but not displaced
3- complete and partial displacement
4- complete and very displaced
Management of an intertrochentaric NOF
Dynamic hip screw
Management of a sub trochentaric NOF
Intramedullary nail
Managment of a undisplaced intracapsular NOF
Internal fixation with cannulated hip screw
Managment of a displaced NOF
Hip arthroplasty
If fit, we’ll and previously able to mobilise- consider total
If significant co morbidities consider hemiarthroplasty
Mx of displaced intracapsular NOF
If well, total arthroplasty
If previous comorbidities, hemi
What x ray films should be taken of NOF
AP and lateral plain film of affected hip, as well as AP pelvis
