What is a signalment?
A detailed description of an animals appearance. Age. Breed. Gender. Reproductive status.
History of present illness?
Main issue, duration, severity, progression, frequency, trigger status, time of day.
Past medical and surgical history?
Other than clued complaint. Illness: when, how long, how treated? Surgery: why, when, prognosis? Medications. Environment. % Diet.
Systems review?
Ask one or more questions about each system.
Vital signs?
Temperature. Pulse. Respiratory rate.(tpr) Mucus membranes. (Mm) capillary refill time. (Crt) weight. Body condition score. (Bcs) - 1-5 scale or 1-9 scale.
