Chief Complaint/ Reason for Seeking Care
This is a brief, spontaneous statement in the person’s own words that describes the reason for the visit. Think of it as the “title” for the story to follow.
Database
The objective and subjective data.Together with the patient’s record and laboratory studies, these elements form the database.
Subjective Data
what the person says about himself or herself during history taking
Objective Data
what you as the health professional observe by inspecting, percussing, palpating, and auscultating during the physical examination
Exacerbation
A worsening. Refers to an increase in the severity of a disease or its signs and symptoms.

Family history
Patient’s family’s past medical history
Precipitating factor
An element that causes or contributes to the occurrence of a disorder.
Predisposing factor
Any conditioning factor that influences both the type and the amount of resources that the individual can elicit to cope with stress. It may be biological, psychological, genetic, or sociocultural.
Remission
Diminution or abatement of the symptoms of a disease; the period during which such diminution occurs.
Risk
A danger or hazard; the probability of suffering harm.
Sign
Any objective evidence of disease or dysfunction. An observable physical phenomenon
Review of systems
Section in the health interview that (1) to evaluates the past and present health state of each body system, (2) to double-checks in case any significant data were omitted in the Present Illness section, and (3) to evaluates health promotion practices.
