What are common manifestations of CD4>500
Acute retroviral syndrome (HIV mono) Persistent generalized lymphadenopathy Aseptic meningitis HIV nervous system disease (peripheral neuropathy) Thrombocytopenia
What is acute retroviral syndrome?
HIV mono, fever malaise, myalgia, 40-70% new infections
Avg. duration: 1-8 wks
Highly infectious during the early part of the disease so don’t miss this
- Ab test is negative early on!!

When does Pneumocystis pneumonia occur?
200 CD4
To whom should we offer HAART treatment?
CD4 500 moderately
Pregnant, Nephropathy, HBV coinfection
HAART therapy is forever so have to be cautious
What are current HAART therapies?
2 NRTIs, NNRTI
PI, 2 NRTI
ISTI, 2 NRTI
Adverse effects of HAART?
HA, Fatigue, malaise, GI disturbances, Metabolic disorders, Drug interactions
Neurologic (Dysesthesias), Sensory neuropathy, Neuropsychiatric
Marrow Suppression
IRIS
What is IRIS?
As HAART is treated on patients with a very low CD4 count can see a over attack on the bug causing local and systemic inflammation
Worse: PCP, M. Tb, MAI, KS, crypto, toxoplasmosis, PML
How effective are condoms at decreasing HIV trans?
Reduces transmission 80%
How effective is HAART at reducing transmission?
REduces it a ton (very good)
What is PrEP?
Prophylaxis antivirals to help combat infection
REduced 93% with good adherence, 43% reduction overall
What is IPERGAY?
On demand PrEP 2 tablets of antivirals, 1 tablet before and 1 tablet after sex
