Cranial Nerve Exam
1- olfactory
2- visual acuity
3,4,6- EOM (H)
5- Motor and sensory (3 areas of face, temporalis/ master)
7- 2 per hemisphere (raise brows, close eyes tight, smile, puff out cheeks)
8- close eyes, finger rub
9, 10- tongue out, say ah, swallow (uvula midline, palate raises)
11- shrug shoulders, turn head (against resistance)
12- tongue out, side to side
Upper Extremities
Sensory Light touch (eyes closed, 2 points) Sensory Vibratory (bony prominence) Sensory Proprioception (eyes closed, up/down/neutral) Radial Pulse Shoulder (1 strength, 2 ROM) Elbow (1 strength, 2 ROM) Wrist (1 strength, 2 ROM) Hand (1 strength, 2 ROM)
Testing Reflexes (7 total)
Biceps Brachioradialis Triceps Patellar Achilles *only unilateral required *Babinski and Clonus in after abd LE exam
Lower Extremity after abdominal exam
Pitting edema (BILATERAL) Dorsalis Pedis Pulse (BILATERAL) Ankle (1 strength, 2 ROM) Foot (1 strength, 2 ROM) Babinski Reflex (unilateral) Test for Clonus (unilateral) *last 2 unilateral Sit up for hip strength
Standing Structural Exam Requirements
