What is Hypermobility
A range of Connective tissue disorders charecterised by increased range of movement and laxity of 1 or more joint
What is HSD Hypermobility Spectrum Disorder
Hypermobility Spectrum Disorder (HSD) is a broad category of joint hypermobility
Symptoms include:
Tiredness, even after rest
Pain and stiffness in your joints or muscles
Sprains and strains,
Joint dislocations,
Poor balance or coordination,
Thin stretchy skin
Bladder or bowel problems
Some people with joint hypermobility syndrome are
asymptomatic.
How can Hypermobility be screened in clinical practice
The Beighton Scoring System measures joint hypermobility on a 9-point scale. The joints assessed are:
Knuckle of both little/fifth/pinky fingers = Can the fifth finger be bent/lifted upwards at the knuckle to go back beyond 90 degrees = 1 point for each hand
Base of both thumbs - arms outstrecthched, palms down with wrist bent downward - can the thumb be pushed back to touch the forearm = 1 point for each thumb
Elbows - arms outstretched, palms facing upwards, does the elbow extend (bend too far) upwards more than an extra 10 degrees beyond a normal position = 1 point for each arm
Knees - knees locked (bent backwards as far as possible), does the lower part of either leg extend more than 10 degrees forward - 1 point each leg
Spine - Can you bend forward and place the palms of your hands flat on the floor in front of your feet without bending your knees - 1 point

What does the Lower Limb assesment score consist of
12 point scoring system/Posotive Score is equal to or greater than 7/12
for each limb
- Hip flexion - mid thigh reaches chest
- Hip abduction - outer knee touches bed
- Knee hyperextension - Heel 3cm off bed
- Knee anterior draw - +ve draw
- Knee rotation - greater than 2cm total Tibial Tubercle Movement
- Ankle dorsiflexion - greater than 15 degrees
- Ankle anterior draw - +ve draw
- Subtalar inversion - greater than 45 degree inversion
- Midtarsal inversion - greater than 45 degree inversion
- Midtarsal ab/adduction- greater than 1cm motion up/down
- 1st MTPJ dorsiflexion - greater than 90 degrees
- Subtalar pronation (stance) - Relaxes to end range pronation
