what ms is affected in posterolateral and lateral thoracotomy
serratus anterior
preload
volume of blood returning to the heart, amount filling the ventricle before it contracts. In VAD, it is the volume available to be pumped by the device
Suckdown
when a collapsed ventricle prevents the pump from filling, causing a decrease or loss of cardiac output
suckdown symptoms
Hypotension, ventricular tachycardia, variable VAD speed
preload symptoms
- Light-headedness, dizziness
- Insufficient filling alarm
afterload
resistance the heart has to pump against
Position of pressure transducer for appropriate swan-Ganz measures
- Anatomic calibration point of peripheral arterial pressure is the level of resting catheter tup in the supine position
effect of high transducer
error BP low
effect of low transducer
error BP high
normal RAP
Normal is 0-8 mmHg, usually <5
increased RAP causes
fluid overload, RV failure, tricuspid valve insufficiency or chronic LV failure
decreased RAP causes
hypovolemia or dehydration
normal pulmonary artery wedge pressure
10-20 mmHg
normal CPP
80-100 mmHg
normal ICP for child
0-5 mmHg
normal supine ICP adult
0-10 mmHg
normal ICP adult
5-15 mmHg
intracranial HTN
sustained ICP > 20 mmHg
ICP for TBI pts
<20 mmHg
CPP for TBI pts
60mmHg
