Idk Flashcards
(15 cards)
How would you introduce yourself ?
Say hello, tell them who you are, state your role and gain informed consent
Identify structures of the breast
Nipple- erectile tissue and milk ducts
The areola - pigmented skin which is a target for the baby
Montgomery tubules - Sebaceous glands that lubricate the nipple
Hormone that stimulates milk production and identify its source and target cell ?
The hormone is prolactin and the source is the anterior pituitary gland, acts on lactocytes which are milk making cells that live in the alveoli
Explain the physiological process which stimulates initiation milk production ?
Baby is born, then placenta,
Easteogen and progesterone levels drop, prolactin raises and milk is produced
Give 3 examples of how effective milk production can be achieved and stimulated ?
Massage
Stimulation
Early feeding
Effective emptying
Identify the hormone responsible for the let down reflex
Oxytocin, released from the posterior pituitary gland.
Name the cell oxytocin acts on and what affect it has ?
Affects myoepithilal cells and has a squeezing effect which pushes milk into the ducts
Explain what factors can help or hinder release of this hormone
Helps- senses, touching, smelling or hearing baby cry
Hinders- high cortisol, separation from baby, lack of sleep
Name the whey protein involved in the autocrine production and maintenance of breast milk
Explain how this works ?
FIL feedback inhibitor of lactation, works as a negative feedback control to increase milk production when needed.
When breasts are effectively emptied, this stimulates more milk supply
How should Robyn prepare herself for a feed ?
Get comfortable
Empty bladder
Hand hygiene 5 moments 7 steps hand washing
Ensure privacy
Watch for feeding cues
Breast feeding positions and reasons ?
Rugby hold- if baby prefers a side
Side lie- ideal for post c section or night feeds
Cross body- good for head support
Laidback- relaxed position good for pain relief
CHINS acronym ?
C - CLOSE (skin to skin)
H - HEAD FREE
I - IN LINE
N- NOSE TO NIPPLE
S- SUSTAINABLE
Detail steps taken by Robyn to attach zac ?
open mouth
Lead with chin
Baby to breast
Nipple to soft pallete
Bottom lip curled
Explain signs of good attachment ?
Not painful
No leaking milk
No loud noises
Rhythmic sucking
Baby coming off the breast spontaneously
How will Robyn know her baby has had enough milk ?
Plenty of wet and dirty nappies
8-12 feeds in 24 hours
Baby settled between feeds
Feeding between 5-40 mins
Coming off the breast spontaneously
