Flashcards in Infectious Disease Deck (18):
1

Risk factors for CAP?
Population with highest prevalence?

- COPD, chronic bronchitis, CV disease, DM
- Smokers (invasive pneumococcal infections)
- Alcoholism, neurologic disease (aspiration )
# Most common in elderly
# 60% of hospitalizations are in pts >65

2

CAP pathogens?

- Strep pneumo most common (also most frequent cause of bacteremic PNA from community)
- H flu, Moraxella
- GNR less common (enterics in chronic liver disease, alcoholism)
- Klebsiella = alcoholism
- Pseudomonas = structural lung disease, chronic steroids
- Staph aureus = think if CAP follows influenza, cavitary PNA, IVDU, recent MRSA skin/soft tissue infection

3

CAP: commonly encountered pathogens in alcoholism?

Strep pneumo, oral anaerobes, Klebsiella, Acinetobacter, M. tuberculosis

4

CAP: commonly encountered pathogens in COPD +/- smoking?

H flu, Pseudomonas, Legionella, Strep pneumo, Moraxella, Chlamydophila

5

CAP: commonly encountered pathogens after exposure to bat or bird droppings?

Histoplasma capsulatum

6

CAP: commonly encountered pathogens after exposure to birds

Chlamydophila psittaci

7

CAP: commonly encountered pathogens after exposure to rabbits?

Francisella tularensis

8

CAP: commonly encountered pathogens after exposure to farm animals or parturient (about to give birth) cats?

Coxiella burnetii

9

CAP: commonly encountered pathogens after exposure to rodent excreta?

Hantavirus

10

CAP: commonly encountered pathogens in early HIV infection? Late HIV infection?

# Strep pneumo, H flu, M tuberculosis
# Same as above plus Pneumocystis jirovecii, Cryptococcus, Histoplasma, Aspergillus, atypical mycobacteria (esp Mycobacterium kansasii), P aeruginosa

11

CAP: commonly encountered pathogens after hotel or cruise ship stay in previous 2 weeks?

Legionella

12

CAP: commonly encountered pathogens after travel/residence in SW US

Coccidioides, hantavirus

13

CAP: commonly encountered pathogens after travel/residence in Southeast or East Asia?

Burkholderia pseudomallei

14

CAP: commonly encountered pathogens after influenza activity in community?

Influenza, Strep pneumo, Staph aureus, H flu

15

CAP: commonly encountered pathogens in IVDU?

Staph aureus, anaerobes, M tuberculosis, Strep pneumo

16

CAP: commonly encountered pathogens with endobronchial obstruction?

Anaerobes, Strep pneumo, H flu, Staph aureus

17

CAP: commonly encountered pathogens in bronchiectasis or CF?

Burkholderia cepacia, Pseudomonas, Staph aureus

18

CAP: commonly encountered pathogens in bioterrorism?

Bacillus anthracis, Yersinia pestis, Francislle tularensis
