A
Azithromycin 500mg PO once daily for 5 days
+ amoxicillin if patient has been on antibiotics in the past 3 months or is at risk of a resistant organism.

A
Oral metronidazole is recommended as initial treatment.
Consider adding IV vancomycin for more severe illness.
