Plaque
A solid elevation of the skin >1 cm in diameter that usually extends into the deeper skin layers.
Nodule
A small circumscribed elevation of the epidermis filled with purulent fluid.
Pustule
A small circumscribed elevation of the epidermis containing clear fluid <1 cm in diameter.
Vesicle
A circumscribed elevation of the epidermis containing clear fluid >1 cm diameter.
Bulla
A circumscribed, raised lesion consisting of dermal edema and is also known as hives or urticaria. Typically last <24 hours.
Wheal
Loss of epidermal or mucosal epithelium
Erosions
Deeper loss of the epidermis and at least the upper dermis
Ulcers
Non-blanchable punctuate foci of hemorrhage
Petechiae
Non-blanchable, raised and palpable
Purpura
Non-blanchable, larger areas or purpura
Ecchymoses
Rash that frequently affects the scalp, extensor, surfaces of the elbows and knees, umbilicus, and the gluteal cleft
Psoriasis
Skin condition that is patchy and isolated or may group around the distal extremities and face, particularly around the eyes and mouth
Vitiligo
Lesions on sun-exposed skin of the face, especially the forehead, nose, and the ear
Discoid lupus erythematosus
Skin containing a high density of apocrine glands, including the axillae, groin, and under the the breasts
Hidradenitis suppurativa
