Report your suspicions immediately
- You are a mandated reporter.
- Even if you’re not sure, it’s better to overreport than underreport.
- Reporting ensures the situation is investigated to protect the patient.
- Documentation is also critical — describe what you observed factually (e.g., location, size, behavior), without assumptions.
