Tell me the process to identify patient with high risk for fall in the department. Any specific assessment tool?
-Upon admission
- when condition changes ( including transfer to other department)
- risk taking behaviour noted
- Fall happened
- reassessment every Mon Thursday
- using Morse fall risk assessment)
Can you share with me how do you manage patient with high risk for FALL.
-Implement standard fall precaution measures (e. maintain sufficient lighting and clear pathway, adjust bed height to lowest, keep call bell within easy reach)
-Restrictive bed rail as needed; Structured rounding - assess & address pain, offer assistance in toileting, position patient into comfortable position, keep belongings within reach;
-Tag patient with “fall risk” wrist tag-
-Close observation (e.g. cohort in fall risk cubicle unless the patient requires isolation)
-Use fall prevention technology (e.g. bed exit alarm, floor alarm) as indicated
-Advice patient/family on fall prevention measures. Patient/family agreed on the plan of care.;
-Encourage family to stay at the bedside, if applicable.
-Advise family to bring appropriate footwear
-Monitor postural hypotension and provide advice
