The phases Nursing Diagnosis
assessment ( data, vitals, past hx), planning, nursing diagnosis(specific problem needing to be addressed) , implementation, evaluation
orthopnea
inability to breathe expect in upright position
crackles
abnormal breathing sounds, high-pitched, heard during inspiration, not cleared by couch, heard at base of the lung
renal diet
kidneys can’t filter off electrolytes and proteins. low sodium, potassium(avocados, bananas, oranges, raisins), phosphorus( whole grain breads, dark sodas, nut and sunflower seeds), protein
total parental nutrition
IV high in glucose, vitamins, minerals, protein, and lipids. requires blood glucose monitoring 4-6 hours
NG tube
check pH, gives meds, feeds, and empties stomach
foot care
don’t cut with scissors, elevate feet
oral care
pt must bu up in bed ( fowers position)
incentive spirometer
pt to achieve maximum voluntary lung expansion and prevents atelectasis, 10 per hour, inhale slowly, sit in semi fowers position, cough after procedure
pulse oximetry
measures the pt arterial oxygen saturation to detect hypoxemia

use foot to push against something. type of strength training in which the joint angle and muscle length do not change during contraction

used with pathogens transmitted by infectious droplet, happens during coughing or sneezing, use mask if puts is being transported, private room wit door closed
aseptic technique
is a method designed to prevent contamination from microorganisms. medical: clean surgical: sterile
