Q

What are the 5 reasons for isolating supplies using the LOTO system

A
  1. To allow maintenance with out hazard to maintainers
  2. To isolate equipment that is defective and may be dangerous
  3. Where equipment has been removed
  4. Where supplies have been provided but equipment has not been fitted
  5. prevent selection of mode of equipment operation which may be unavailable or dangerous

Q

State the LOTO procedure for REPLACING Fuses. x9

A
  1. The person who conducted the LOTO should be the person who reinstates equipment / system back in.
  2. If the original person is not available the Authorising individual can brief another competent person to restore supplies.
  3. Obtain permission to restore supplies
  4. Ensure equipment is switched off
  5. Obtain fuses for correct circuit from LOTO box and remove Part C LOTO label
  6. Remove Part B of the LOTO label
  7. Remove Part A of the LOTO
  8. Replace fuses
  9. Hand LOTO labels ABC to the authorising person to be destroyed.

inform users
fill out index sheet and collect next number in sequence serial number
complete relevant sections of LOTO safety certificate and sign
Complete LOTO labels
Make sure equipment is switched off
remove fuse and place Part A LOTO label to empty fuse holder
Place B Label on the LOTO board and C Label with fuse/ keys in box

17
Q

What PPE should be worn whilst working in the galley?

A

DMS boots
overalls with sleeves rolled down
Chefs paper hat

18
Q

Name the dangerous components and their associated hazards found on board HM Ships?

A

Tantalum capacitors
selenium rectifiers
Pentane
Radioactive Materials

19
Q

What are the x4 precautions are to be taken when using Penetone TPC?

A

Keep away from naked flames
supply and exhaust fans are to be running
never to be sprayed directly onto live electrical equipment
always wear rubber gloves

20
Q

How are all galley supplies isolated in the event of a fire?

A

Red isolation switches fitted outside

21
Q

What is the biggest fire hazard within the galley?

A

chip pan friers hot oil

22
Q

What specific PPE should you wear when working on tantalum capacitors?

A

face protection

23
Q

What are the precautions to be taken when working on dead equipment?

A

Isolate and LOTO
Prove equipment is dead
Discharge any capacitors in the circuit
if required fit shorting straps to prevent capacitors from re-charging

24
Q

How would You prove that a circuit is DEAD before working on it?

A

By measuring for zero vaults between phases and between phase and earth

25
Q

When you replace Fuses at the Fuse Panel. How do you prove Electrical Switches are open, or not?

A

by using a voltage tester and measure for 0 volts across the empty fuse ways

26
Q

List the five examples of HUMAN FAILURE that could lead to Electric Shock?

A
  1. failure to observe proper safety precautions
  2. shoddy work
  3. failure to properly test portable equipment
  4. familiarity
  5. standing in damp surroundings
27
Q

What is a CP? And What is the definition of one?

A

CP (competent person) is a person who has sufficient technical knowledge
or experience to enable the person to avoid danger from electrical hazards.
