Hansen’s disease
Criteria for leprosy
- 1/3 or 2/3 criteria
- retrieved defaulters with signs of active disease
- relapsed patients who previously completed a full course treatment
Diagnoses of leprosy
- hypopigmented or redish skin lesion with loss of sensation
- involvement of peripheral nerves
- skin smear positive for acid fast bacilli
incubation period for M. leprae
5 years ( 6 months to 20 yrs)

Superficial peripheral nerves
Anterior chamber of eyes
Testes
Single or few, asymmetrical, well-defined hypopigmentation, erythematous or copper-colored patches with raised borders
mild form, no satellite lesions
Tuberculoid Leprosy
Few, asymmetrical, well- demarcated, dry plaques, may be annular, may be scaly, clearly defined outer borders
Borderline Tuberculoid Leprosy
more numerous lesions than tuberculoid ones > 3 and <10 lesions
asymmetrical, less well demarcated, shiny plaques
Mid borderline Leprosy
numerous (>5), smaller roughly symmetrical, shiny macules, papules, nodules and plaques with sloping edges
annular punched out lesions
Borderline Lepromatous Leprosy
Very numerous, symmetrically distributed, erythematous or copper-colored shiny macules, papules and nodules
(+) infiltration in the skin
(+) lepra bonita
Lepromatous leprosy
Multibacillary
> 5 skin lesions
+ AFB in skin smear
1 nerve trunk involved
Paucibacillary
< 5 skin lesions
No AFB in skin smear
0-1 nerve trunk involved
Component of MDT
Rifampicin
Clofazimine
Dapsone
Patient becomes non infectious in ______
1 month
Treatment of paucibacillary leprosy
Rifampicin 600 mg once a month
Dapsone 100 mg for 6 months
Treatment of Multibacillary leprosy
Rifampicin 600 mg and Clofazimine 300 mg once a month
Dapsone 100 mg and Clofazimine 50 mg daily for 12 months or until skin smears show no more bacilli
Side effects of MDT
Rifampicin ( reddish urine)
Clofazimine ( dark brown skin)
Dapsone ( hemolytic anemia)
