Docusate sodium
Colace
Class: stool softener
MOA: allows easier passage of stool
Indication: constipation hold: diarrhea
Side effects: nausea, diarrhea, throat irritation
Asses before Med: pts last BM, cause of constipation
Eval after Med: if pt was able to have a BM
Pt teaching: can take up to 3 days to soften stools, increase fiber and fluid
Omeprazole
Prilosec
Class: antiulcer, proton pump inhibitor
MOA: lower secretions of acid in the stomach
Indication: acid reflex, GERD, Gi bleed hold: life threatening reactions
Side effects: head ache, dizziness
Asses before Med: asses bowels sounds, asses if pt has heartburn or epigastric pain
Eval after Med: decrease in heartburn or epigastric pain
Pt teaching for Med: take in the AM before breakfast, swallow whole, avoid alcohol
Benazepril
Lotensin
Class: antihypertensive, ACE inhibitor
MOA: lower body prevent vasoconstriction
Indication: hypertension hold: hypotension
Side effects: low no, dizziness, fainting
Asses before: asses bp and heart rate
Eval after Med: reassess bp and heart rate
Pt teaching: repot dry cough, sob, swelling of airways, dizziness
Miconazole
Monistat
Class: antifungal
MOA: prevents fungal or yeast growth
Indication: fungal/yeast infection Hold: allergic reaction to rx
Side effects: headache, vaginal itchiness, and or burning.
Asses before rx: asses vagina condition and infection
Eval after rx: improvement of vagina condition and infection
Client pt teaching: empty bladder before giving the medication to pt, lay flat 10-15 min after given
Acetaminophen
Tylenol
Class: analgesic
MOA: may block pain impulses
Indication: mild aches and fever hold: signs liver failure
Side effects: nausea, vomiting, abd pain
Ass before rx: fever, asses pain, liver function >32g in last 24 hrs
Eval after rx: asses pain and temp for improvement
Pt teaching: don’t take above recommended dose
Oxycondone/acetaminophen
Percocet
Class: opioids analgesic
MOA: increases pain threshold
Indication: moderate to severe pain. Hold: resp <12 or systolic bp <90. Or a drowsy person
Asses before rx: BP and respirations
Eval after rx: assess pain and vital signs
Pt teaching: don’t drive or combine with alcohol. Increase fiber and fluid intake to prevent constipation
Biscodyl
Dulcolax
Class: laxative
MOA: acts directly on intestines by increasing motor activity
Indication: constipation hold: diarrhea
Side effects: nausea, vomiting, stomach cramps, diarrhea
Asses before rx: know when last BM was, bowel sounds, and find out what possible caused diarrhea.
Eval after rx: if your pt is able to have a BM
Pt teaching: lay in sims position for 5-15 min. Increase fiber and fluid intake
Furosemide
Lasix
Class: diuretic
MOA: helps resorption of fluid by allowing kidneys to excrete it
Indications: pulmonary edema, hypertension. Hold: hypotension, hypokalemia
Asses before rx: bp, electrolyte panel
Eval after rx: bp, electrolyte, weight, fluid balance
Pt teaching: may cause sleepiness take in the AM
Diphenhydramine
Benadryl
Class: antihistamine
Indications: allergies and insomnia
Side effects: drowsiness, dry mouth and nose
Ass before rx: signs of allergies hold: if pt is too drowsy
Pt teaching: don’t drive and combine with alcohol
Simvastatin
Zocor
Class: antilipidemic
Moa: prevents body from making cholesterol
Indications: increased cholesterol hold: abn hepatic studies
Side effects: headache, muscle cramps, diarrhea
Asses before rx: hepatic studies
Eval after: decrease in cholesterol after months of therapy
Pt teaching: do regular labs to monitor cholesterol and promote low cholesterol diet and exercise (high cholesterol food: eggs, fish, meat, butter)
