Acetylsalicylic Acid (Aspirin)

324 mg / 4 x 81 mg baby aspirin PO if not taken during previous 24.

Adenosine (Adenocard)

6 mg rapid IV followed by 10-20 mL NS flush
if no response in 2 minutes, 12 mg

Abuterol (Proventil)

2.5 mg by nebulizer up to 7.5 mg.
continuous, if in moderate to severe distress, or respiratory failure is impending (Asthma/COPD)

Amiodarone (Cardorone) for V-Tach c pulse

V-Tach c pulse: 150 mg in 100 ml NS over 10 minutes. May repeat once.

Atropine Sulfate for bradycardia

0.5 mg IV. Rpt in 5 minutes if pt remains symptomatic

Calcium Chloride 10%

1 gm IV. Slow admin for pts c a palpable pulse when treating crush syndrome or pts c profound hypotension after the admin of diltiazem. IVP when pt is in cardiac arrest

Dextrose 50%, 25%, 10%

>12 yo: 25 gm slow IV. rpt once if BGL remains <70 mg/dl

Diazepam (Valium)

2-10 mg IV to 20 mg.

Diltiazem (Cardizem)

0.25 mg/kg IV over 2 minutes. May repeat in 15 minutes with 0.35 mg/kg

Diphenhydramine HCL (Benadryl)

25-75 mg, slow IV/IM

Dopamine (Intropin)

5-20 ug/kg/min infusion

Enalaprilat (Vasotec)

1.25 mg IV

Epinephrine HCL 1:1000 and 1:10,000 for Cardiac Arrest

1:10,000 1 mg IV/IO every 3-5 for duration of arrest.
1:1,000 2 mg ET diluted in 5 ml NS every 3-5 minutes

Etomidate (Amidate)

0.3 mg/kg IV/IO over 1 minute, up to 30 mg. May repeat once.

Fentanyl (Sublimaze)

25-50 mcg IV per dose every 5 minutes to 200 mcg.
50-100 mcg IV for burns
25 mcg for elderly or under 70 kg weight

Furosemide (Lasix)

20-40 mg slow IV MEDICAL CONTROL REQUIRED

Glucagon HCL

Hypoglycemia: 1 mg IN/IM
Calcium Channel / Beta Blocker OD: 1 mg IV every 5 up to 3 mg
Allergic Rxn / Anaphylaxis not responding to Epi: 2 mg IVP

Glucose, oral

25-50 gm PO may repeat once

Haloperidol (Haldol)

5 mg IM. Over 65yo 2.5 IM

Hydrocortisone (Solu-Cortef)

100 mg IV/IO/IM

Hydroxocobalamin (Cyanokit)

5 g infused over 15 minutes in 100 ml. MED CONTROL for another 5.

Ipratropium Bromide (Atrovent)

2.5 ml (500 mcg) mixed with 2.5 mg Albuterol via neb

Ketamine (Ketalar)

0.2 mg/kg IV/IO/IM administered over 1 minute. Repeat once in 5 minutes for pain. 3rd one for Crush Injuries.

Labetalol (Normodyne)

10-20 mg slow IV over 1-2 minutes repeated in 5 minutes to a total of three doses, as long as SBP is greater than >180 mmHg

Lidocaine HCL (Xylocaine) for pVT/VF

1 mg/kg IV/IO may be repeated every 5 minutes up to 3 times

Magnesium Sulfate for Torsades de pointes (pulseless)

2 g slow IV/IO. Mix 2 g in 10 ml NS and admin over 2 min.

MethylPrednisolone (Solu-Medrol)

125 mg IV/IM

Midazolam HCL (Versed) for SZ/Eclampsia/Excited Delirium/Nerve Agent, Organophosphate, or Carbamate poisoning

10 mg IN, 5mg ea nostril. MED CONTROL FOR MORE
5-10 mg IM
2-5 mg IV/IO every 2 minute to relief or 10 mg.

Morphine Sulfate

Pain Mgt./Burns. 2mg IV repeat prn for relief to 10 mg. May repeat after 10 minutes.

Cardiac Cx Pain: 2 mg IV, up to 10 mg.

Naloxone HCL (Narcan)

2 mg IV/IN or IM. If no response within 5 minutes, repeat 4 mg IV/IN and titrate to effect thereafter.
