Primary dysmenorrhea
Idiopathic Onset shortly after menarche; usually improves with age Most common in nulliparous women Possibly due to excessive prostaglandins (which cause strong contractions, which causes ischemia)
Secondary dysmenorrhea
Due to underlying pathology Onset later in life May involve history of: PID, IUD, endometriosis, hormone imbalance, narrow cervix, uterine malposition, etc
Primary Dysmenorrhea: Sx
Pain prior to and during menstruation, peaking within 24-36 hours Suprapubic, crampy pain Breast swelling and tenderness HA etc etc Pain can radiate to thigh, groin, vulva
Dysmennorhea: Tx Goals
Decrease Sx Reduce pain and edema Improve circulation and blood flow around pelvis Decrease spasms, adhesions, HT
