You are the IMT-1 on-call and you have been asked to see a patient with a nasogastric tube, who has started coughing after feeding. You note that the x-ray undertaken prior to feeding demonstrates that the NG tube is not correctly cited and the x-ray has not been reported.

immediately ask the nurse to stop the feeding
assess ae
if safe to do so extract the ng tube
and the send for bloods nd repeat cxr - as risk of aspiration pneumonia
do duty of candour to the patient
fill a clinical incident form
this is NHS ‘never event’
