Q

Grading system for a hip extension supine

A

5/5: pelvis & back elevated as one unit as heel lifted approximately 24 inches off table

4/5: hip flexes less than or equal to 30° before hip and back elevate

3/5: strong resistance to elevation of limb to 24 inches off table without elevation of pelvis

2/5: minimal resistance (more than weight of limb) to 24 inches off table

No grade 1 or zero

A

Position: prone on table arms at side

Instruction: raise your head, shoulders, and chest from the table as high as you can

Grade 2: partial range

Grade 1: trace contraction in thoracic or lumbar extensors

Great 0: zer0 contractile activity

16
Q

Trunk extension

A

Position: prone

Resistance: changes with arm position

Stabilization: therapist uses bodyweight at patients ankles or hips

17
Q

Trunk flexion grading

A

Grade 5: no assist, hand behind head, clear inferior angles

Grade 4: no assist, hands crossed over chest, clear inferior angles

Grade 3: no assist, hand straight forward, clear inferior angles

Grade 2: raise head and upper trunk, scapula do not clear table, And palpate abdominals contract

  • Assisted and active forward lean with rib cage depression
  • Cough in assisted forward lean with rib cage depression

Grade 1: cough or perform an assisted forward lean without rib cage depression, but palpable contraction
-Unable cough but palpate rectus contraction

Grade 0: zero detectable activity

18
Q

Trunk rotation (internal and extternal oblique

Position, action, instructions for grade 5-3.

A

Position: supine with legs straight; for trace hips flexed feet flat

Resistance: changes with arm position

Stabilization: bodyweight

Action: curl up performed movements bilaterally

Instructions grade 5/4/3: lift your head and shoulders from the table, taking your right elbow toward your left knee. Then that lift your head and shoulders from the table, taking your left elbow toward your right knee

Criteria G5: with hands clasped behind head must clear scapula on side being rotated away from

Criteria G4: with arms crossed must clear scapula on side being rotated away from

Criteria G3: with arms outstretched across plan of body can raise scapula off table

19
Q

Grade 3 trunk extension position, instruction, and grade

A

Position: prone off the table tunable line arms outside

Instruction: raise your head shoulders and chest from the table as high as you can

Grade 3 lumbar/thoracic raises up with arms at side

20
Q

Trunk flexion

A

Position: supine

Resistance: changes with arm position

Stabilization: if hip weakness stabilize pelvis

Instruction: tuck your chin and bring your head, shoulders, and arms off the table curling up into a sit up

21
Q

Thoracic Trunk extension position, instruction, grades 4 and 5.

A

Position: thoracic off table to nipple line with hands clasped behind head.

Instruction: raise head, shoulders, and chest to table level

Grade 5: thoracic raises to horizontal+ with ease

Grade 4: thoracic raises to horizontal with difficulty

22
Q

Trunk rotation instructions and grading criteria for grades 2-0

A

Instruction grade 2: Lift your head and reach toward your knee

Criteria G2: arm straight unable to clear inferior angle; depression of rib cage must occur

Instruction grade 1: therapist assists by supporting patients head to decrease weight that abdominals must lift.
Say Try to lift up and turn your right then left

Criteria G1: see or palpate contraction

Criteria G0: no response

23
Q

Lumbar trunk extension position, instructions, and grades five and four.

A

Patient position: Lumbar on table with hands clasped behind head

instruction: raise your head, shoulders, and chest off the table. Come up as high as you can

Grade 5: lumbar raises up clears up umbilicus and locks without effort or lag

Grade 4: lumbar raises up clears umbilicus but with effort and wavering
