Main categories of MND
A
ALS - affects both UMN and LMN.
LMN - Flaccid, weakness, wasting & fasciculations
UMN - Spastic paralysis, hyper-reflexia, Babinski sign and pyramidal weakness (extensor more weak in UL, Flexors weakened more in LL).
25% of ALS begin as PMA and develop into ALS.
Primary Lateral Sclerosis - UMN signs only.
Progressive Muscular Atrophy - LMN signs. Distal before proximal muscles. Best prognosis.
Progressive Bulbar Palsy - Worst prognosis
Palsy of the tongue, muscles of chewing and swallowing and facial muscles - due to loss of brainstem motor nuclei.
