What questions are in the depression screen? When should they be asked?

Answer 1

1) During the last month, have you been feeling down, depressed, or hopeless?
2) During the last month have you been troubled by having little interest or pleasure in doing things?

If the answer is “yes” to one of these, ask the following 3 questions:

1) Have you bothered by feelings of worthlessness
2) poor concentration
3) or thoughts of death

These should be asked in anyone at risk of depression including those with a past history of depression or those suffering from a chronic physical impairment with functional impairment
