What causes acute pharyngitis/tonsilitis?
Viral MC: Adenovirus, rhinovirus, enterovirus, EBV, RSV, influenza A & B, HZV
Bacterial: GABHS
What are s/s of pharyngitis/tonsilitis?
Sore throat
Pain w/ swallowing or w/ phonation
How do you treat pharyngitis/tonsilitis?
Sx: Fluids, warm gargles, anesthetics, lozenges, NSAIDs
Abx if S.pyogenes: PCN, Amox
If PCN allergic –> macrolides
What causes aphthous ulcers (canker sores)?
May be a/w HHV6
What are s/s of aphthous ulcers (canker sores)?
Small, round/oval painful ulcers (yellow, white, or grey centers) w/ erythematous halos
MC on buccal or labial mucosa
How do you treat aphthous ulcers?
Topical analgesics
Topical PO steroids (triamcinolone, fluocinonide), vit B&C
If recurrent –> cimetidine
What causes laryngitis?
Viral infection MC - adenovirus, rhinovirus, influenza, RSV, parainfluenza
Bacterial: M. catarrhalis & mycoplasma pneumo
Trauma
What are the s/s of laryngitis?
Hoarseness = hallmark*
Aphonia*
Pharyngitis, rhinitis, cough
How do you treat laryngitis?
vocal rest, warm gargles, anesthetics, lozenges, fluids
What causes oral candidiasis?
Candida albicans
What are the s/s of oral candidiasis?
Mouth or throat pain
How do you diagnose oral candidiasis?
Clinical: white curd-like plaques (+/- leave behind erythema/bleeds if scraped)
KOH smear: budding yeast/pseudohyphae
How do you treat oral candidiasis?
Nystatin liquid = tx of choice
Clotrimazole troches, oral fluconazole
What is oral leukoplakia?
PreCA hyperkeratosis due to chronic irritation (tobacco, cigarettes, Etoh, dentures)
6% show dysplasia or SCC
What are s/s of oral leukoplakia?
Painless white patchy lesion that cannot be scraped off
How do you treat oral leukoplakia?
Cryotherapy, laser ablation
Biopsy to assess from CA risk
What is sialadenitis?
Bacterial inf of parotid or submandibular salivary glands
What causes sialadenitis?
S. aureus MC
Dehydration, chronic illness
What are s/s of sialadenitis?
- pain, swelling, erythema near gland (esp. w/ meals)
- tenderness at the duct opening (+/- pus when massaged)
- pain, dysphagia, trismus –> fever, chills (if severe)
How do you diagnose sialadenitis?
CT
How do you treat sialadenitis?
Sialogogues (tart candy, lemon drops)
Antistaphylococcus (dicloxacillin or nafcillin) + metronidazole or clinda if severe
What causes oral herpes?
HSV1
MC btwn 6mos-5yo
What are s/s of oral herpes?
Sudden onset fever
Anorexia
Gingivitis
Grey/yellow vesicles on mucosa, tongue, lips
How do you treat oral herpes?
Self-limiting
Acyclovir if severe
