paracetamol, if not working give topical NSAIDs

reversibly (apart from aspirin, which irreversibly inhibits COX1) inhibit COX2 enzyme, necessary for prostaglandins
- ADRs = from COX1 inhibition

pain relief, not anti-inflammatory, only works in CNS so blocks pain conduction

How to treat RA?
A
NSAIDs good for symptomatic relief (explain to them they shouldn’t take all the time), prednisolone for flare ups - can’t be given for long. First line is DMARDs - methotrexate,, sulfalsalazine
Biological agents = if methotrexate isn’t working; inflictamab

an anti-TNF, therefore has ADRs; serious infection, TB reactivation, Hep B
