What is the best initial treatment for rhabdomyolysis?
The initial step in management of rhabdomyolysis is intravenous fluid rescuscitation
Limping and pain in groin and thigh in a young boy
Legg-Calve-Perthes disease (avascular necrosis of the proximal femur)
Dx: hip radiographs
PE: limited passive movement of the affected hip joint as well as limited abductiona and internal rotation
Tx: supportive or surgical
Lightheadedness, confusion, chest pain and SOB after a fracture of a long bone (femur)
Fat embolism syndrome
is most often caused by long bone fractures and causes hypoxemia, altered mental status and a petechial rash
What test is the gold standard for diagnosing avascular necrosis of bone?
MRI
*avascular necrosis of the hip presents with pain in abduction and internal rotation and a severely limited range of motion in all directions.
here are some awesome clues to this diagnosis:
History of cancer, weeks of back pain, loewr extremity weakness and loss of sensation in the saddle region of the legs.
Epidural spinal cord compression is most commonly a complication of metastatic cancer to the spine.
back pain leads to lower extremity weakness or paraplegia.
Initial management is with high-dose corticosteroids (dexamethasone) to reduce inflammation and edema- hence decreasing compression on the spine.
what is the treatment for any cobb angle <20?
observation is the management of choice for patients with a Cobb angle of less than 20 degrees.
what is the first line treatment for ankylosing spondylitis?
NSAIDs (indomethacin)
ankylosin spondylitis is a progressiv, inflamatory spondyloarthritis. It most commonly involves the joints of the axial spint, producing pain and a limited ROM.
A patient develops malignant hyperthermia at the onset of anesthesia. Do you continue with the procedure?
No, you dummy. Administer dantrolene IV and terminate the procedure.
