finding the UCL
bend elbow, palpate medial epi, then move a lil bit distally
stability test, valgus and varus stress testing
@ 0 degree, not hyper
@ 20 degrees
pronate them (decreases carrying angle, reduces vektor of motion)
place wrist between elbow and body
with both hands give a lil shake to make laxity
then top hand will give valgus and varus stretch/tension
UCL injury possibly, anterior bundle?
moving valgus stress test
pt @ 90 90
be in front or off to the side
move arm through arc of motion like throwing a ball
this will stress inside of elbow
push elbow anteriorly slightly
while performing 70-100 degrees of flexion
milking maneuver , UCL stress test
pt in supine
they at 90 90
they rest in pit of your forearm
supinate them a bit
grab their thumb
extend your own elbow
making ER of their arm
our elbow will push them in a valgus (their elbow will move anterior)
LCL push off test , lateral elbow rotary instability or a posterior lateral rotary instability and possibly a dislocation
pt seated
supinate hands along hips, flat on table
“push themselves off table”
how elbow dislocations occur, you land with an axial load in supination and a posterior force, and the radius may dislocate or the ulna may dislocate posteriorly
total instability and no ligament integrity, will dislocate
partial damage, may feel a clunk or pain or apprehension
cozens sign, active test
lateral epicondylagia
they are at 90 degrees elbow flexion at side
they resist you trying to move their hand out of extension and they resist you trying to move their elbow into elbow extension
they will try and stay in wrist extension and elbow flexion
testing the ability of the wrist extensors to maintain position
mills test, passive test
lateral epicondylagia
they will stretch their extensors while in elbow extension and pronation
they can do it or we can do it passively
they report pain in lateral epicondyle
Maudsleys test, 3rd digit
extensor carpi radialis brevis as source of lat epicondylagia
resisted 3rd digit extension
they will extend their arm forward
if lateral elbow pain positive
medial epicondyle shear test , active test
medial epicondylitis 1
seated
supinate and slightly flexed pt position
their palm is facing ceiling
they will try and maintain a wrist flexed position
they will try and maintain a slightly elbow flexed position
you will try and actively pull them out
if they have medial pain, positive
medial epicondylitis test 2, passive test
they stretch their wrist flexors in elbow straightened position
either PT or pt
distal biceps tendon special test
hook test
supinate arm
gentle ISO into elbow flexion
then hook bicep tendon moving medially to lateral
Can you palpate it
palpation of tendon: apparent or not apparent
biceps squeeze test
pt in slight pronation
should see a slight supinate
