Caused by gram negative bacillus Burkholderia pseudomallei
Through skin wounds
South east Asia mainly
Fever cough headache pleuritic pain and generalised myalgia

Fever + severe aching + rash

A

Dengue fever
Abrupt onset fever, malaise, headache, nausea, pain behind eyes, severe backache, prostration , sore throat
Fever subsides then returns
Maculopapula rubelliform rash limbs then trunk
Mosquito bites
Incubation 5-6 days
Symptomatic management

Stepladder fever+ abdominal pain + relative bradycardia

A
Typhoid (early) 
Insidious onset
Headache
Dry cough
Fever gradually increases over 4 days
Aldo pain and constipation
Diarrhoea (pea soup) and rash (rose spots) late 

Rx ciprofloxacin 500mg BD 14 days
