complains of pain and tingling along left lateral forearm, thumb, and first finger. weakness in elbow flexion and wrist extension

wrist extension: extensor carpi radialis longus/brevis, extensor carpi ulnaris (radial N)
reflex: brachioradialis
tingling sensations running down the inside or right leg and foot. weakness of knee extension and ankle that rolls outward
root: L4

tests: foot inversion, slight dorsiflexion- tibialis anterior (deep peroneal N)
reflex: patellar
numbness in back of left calf and side of foot. can’t use clutch in car bc foot can’t press hard
nerve: S1
disc: L5
tests: foot plantar flexion- gastrocnemius and soleus(tibial n)
foot plantar flexion and eversion- peroneus longus/brevis (sup peroneal N)
hip extension- glut max (inferior gluteal)
reflex: achilles
complains of left shoulder pain. may be related to the weakness he has been experiencing in left arm while doing curls at gym
nerve: C5
disc: C4
tests: deltoids- axillary N
biceps-musculocutaneous
reflex: biceps
difficulty facebooking as he types. great difficulty of finger weakness and notices numbness of medial surface of fingers 4,5
nerve: C8
disc: C7
tests: finger flexion- flexor digitorum superficialis, flexor digitorum profundus, lumbricals (median & unlnar)
reflex: none
left foot pain. points to top of foot as site. when he walks his left hip seems more stable than right.
nerve: L5
disc: L4
tests: *foot dorsiflexion- tibialis anterior (deep peroneal)
* big toe dorsiflexion- extensor hallicus (deep peroneal)
* toes 234 dorsiflexion- extensor digitorum longus/brevis (deep peroneal)
* hip abduction- glut med/min (superior gluteal)
reflex: none
right medial elbow tingling sensations. concert pianist and seems weaker when playing
nerve: T1

tests: finger abduction- dorsal interossei (ulnar)
finger adduction- palmar interossei (ulnar)
reflex: none
