A
- Promote the implementation of preventative measures
- Allow for easy reporting of serious incidents.
- To increase awareness of these events in the workforce.

Individual responsibility- taking accountability of actions- practice ethically- don’t blame others
Systemic changes- organisations should routinely examine systems- equipment. procedures & training
develop relationships w/ patient- communicate & actively listen
