Normal findings for dermatologic exam (hair, skin, nails)
Skin is warm, well moisturized, and intact with normal colour for ethnicity. No lesions, rashes, or abnormalities noted. Hair is evenly distributed with normal texture. Nails are smooth, without clubbing, pitting, or discoloration
Normal findings for external eye exam?
Eyes are symmetrical with no swelling, redness, or discharge. Conjunctiva is clear, and sclera is white. No lesions or abnormalities noted on the eyelids or surrounding structures.
Normal findings for liver percussion/ palpation?
Liver is not palpable OR smooth, non-tender, and there are no abnormal masses or enlargement noted.
Normal findings for skin exam?
Skin is warm, well moisturized, and intact with normal colour for ethnicity. No lesions, rashes, or abnormalities noted
NSE normal findings?
- The nails are smooth, pink & healthy with no signs of brittleness or spooning.
- Hair is evenly distributed with normal texture.
- There is no noticeable pallor in the skin or nailbeds, palms, tongue, conjunctiva or rim indicating good nutritional status and no signs of anemia.
Normal findings for thyroid exam?
Upon inspection and palpation, the thyroid gland is not visibly enlarged, and there are no noticeable lumps or asymmetry. There is no tenderness on palpation, and the gland moves symmetrically upon swallowing. No signs of goiter or nodules are present.
Normal findings for nose exam?
The nasal mucosa is pink and moist, with no septal deviation, no polyps, and no signs of inflammation or discharge. Turbinates are non-edematous and symmetrical. Sinuses were not tender on palpation.
Normal findings for inspection of thorax (anteroposterior diameter, chest deformity)?
-symmetric anterior and posterior thorax w/ downward sloping ribs
-trachea + sternum midline
-no thorax abnormalities, masses, swellings
-costal angle 90 degrees
-Consistent skin colour, no discolouration
Normal findings for lung percussion + auscultation?
percussion: resonant throughout
auscultation: breath sounds were vesicular throughout all lung fields, with no wheezes, crackles, or other adventitious sounds.
Normal oropharynx exam?
The oropharynx is clear with no redness, swelling, or exudate. Tonsils are 1+ & there is no visible ulceration or lesions.
Normal gait findings?
The gait is normal, with smooth, coordinated movements while walking. The patient can walk on their toes and heels without difficulty and can perform the toe-to-heel walk in a controlled and stable manner.
