Nose Flashcards
(8 cards)
What are the risks for side-to-side closure on the nose?
a. Elevation of the alar rim
b. Distortion of the nostril
c. Blunting of nasal root concavity
d. Elevation of the nasal tip
e. accentuation of the convexity
What are the aims of repair for a nasal tip closure?
- Restore normal anatomy
- Preserve the soft triangles
- Minimse tip elevation
- Avoid alar elvation
- Avoid nostril size reduction
What factors do you take into account when determining closure technique? (for nasal tip lesions)
- Size and shape of the defect
- Size of the nose and the skin laxity
- Degree of nasal tip ptosis
- Tissue matching
- Patient selection
- e.g cigarette smoking, reducs graft viabilitye
- blood thinners
- geographical proximity of the patient to care
What repair options are available for the nasal tip?
- Side to side closure
- Advancement flaps
* Burow’s exchange - Rotation flaps
* Dorsal nasal
* Double
* Advancement of Inferior rotation of the nasal sidewall) - Transposition flaps
* Rhombic
* Bilobed
* Trilobed and multilobed - Island pedicle flaps
* Subcutaneous
* Myocutaneous
* 2 stage - FTSG
Regarding nasal tip side to side closure:
What are the advantages?
What are the disadvantages?
Advantages:
* Simple closure
* Can be orientated vertically or horizontally
* Can cause some nasal tip elecation if nasal tip ptosis is present
Disadvantages:
* Elevation of the nasal tip
* Elevation of the nasal alar / soft triagles which may distort the nostrils
* Can flatten the nasal tip convexity
When would you use a Burows exchange advancement flap?
What are the advantages and diadvantages?
Ideal for small defects just lateral to the midline
Advantages - good skin match
Disadvantage - possible soft triangle distortion, narrowing of the columella
Please draw a burrows exhange flap on the nose.
What is a dorsal nasal rotation flap?
What are the advantags and disadvantages?
Advantages:
* May repair defects up to 2cm
* Well vascularised flap
