Q

Vitamin A deficiency
Cause
Problems

A
  • Deficiency mainly occurs in fat malabsorption
  • Primarily affects the eyes
  • Night blindness is initial sign
  • Xerophthalmia – drying of conjunctiva / cornea
  • Bitot’s spots – abnormal squamous cell proliferation and keratinization of conjunctiva caused by xerosis (drying of conjunctiva)
  • Blindness may occur due to progressive corneal xerosis and keratomalacia (degeneration of cornea) → corneal clouding
  • Skin – dry skin, flaky dermatitis, follicular hyperplasia

Q

Thiamine deficiency
Which vitamin?
Associated w/ what?
Problems

A

Vitamin B1
•Deficiency mainly occurs in alcoholics
•Dry Beriberi: peripheral neuropathy (sensory and motor; wrist / foot drop, ataxia, paresthesia) and angular stomatitis.
•Wet Beriberi: neuropathy + cardiomyopathy and CHF
•Wernicke-Korsakoff Syndrome
• Wernicke’s encephalopathy is acute: confusion / delirium, disturbance in eye movement (nystagmus / ophthalmoplegia), ataxia, tremors
• Korsakoff’s psychosis is chronic: severe short-term memory impairment and confabulation.

Q

Niacin deficiency
Which vitamin?
Cause
Problems

A
  • Vit B3
  • Deficiency occurs w/ diets low in niacin and tryptophan, such as corn-based diets.
  • Starts w/ weakness, anorexia, and indigestion, followed by the 4 D’s.
  • Pellagra (4 D’s)
  • Dermatitis on sun-exposed areas. Begins as erythema w/ itching / burning. Blebs → bullae → burst. Later the skin becomes hard, rough, cracked, blackish, and brittle.
  • Diarrhea (also dysphagia / inflammation of mouth; tongue may be red / smooth / edematous).
  • Dementia (if untreated)
  • Death

Q
Vit C deficiency
Cause
Molecular problem
Problems / signs (8)
1st sign / sx
A
  • Occurs w/ severe malnutrition and lack of fruits / veggies
  • Impairs collagen synthesis
  • Causes swollen / bleeding gums, perifollicular petechiae, bruising, hyperkeratosis, coiled hair, joint pain, poor wound healing, and fatigue.
  • Gums may be blue / red. Normal gums in areas w/o teeth.
  • 1st sign is usually perifollicular petechiae. 1st symptom is usually fatigue.

Q

Vitamin D deficiency
Cause
2 age counterparts

A
  • Occurs due to fat malabsorption and north
  • Rickets in kids – Hypertrophy of chondrocytes and defective mineralization at growth plate causes short stature, enlarged joints / skull
  • Osteomalacia in adults - impaired mineralization of bone matrix causes bone pain and muscle weakness

22
Q

What 2 things cause muscle spasms / tetany?

23
Q

What 2 things cause muscle weakness?

A

Potassium and Vit D

24
Q

What 2 things cause paresthesias, ataxia, decreased vibratory / position sense, wrist / foot drop?

A

B12 and thiamine

25
What 2 things cause dementia?
B12 and niacin
26
What causes dementia w/ confabulation?
Thiamine (B1)
27
How long is the body reserve for thiamine?
4-10 days
28
What deficiencies are alcoholics at risk of? (5)
Alcoholics are at risk for low B6, folate, B12, and vit C. Poor absorption of thiamine / folate.
29
Deficiencies in elderly (4)
Elderly often have poor access to fruits / veggies (Vit C / folate), meat (zinc), and decreased sun exposure (Vit D)
30
What 4 things cause Mg loss?
Mg loss may occur via urinary loss in alcoholics, diarrhea, diuretics, or diabetes
31
What 2 deficiencies are caused by pancreatic insufficiency?
ADEK and B12 (need protease)
32
Celiac disease (5)
Celiac disease → malabsorption of folate, B12, fat soluble vit (esp D / K), Ca, iron
33
Gastrectomy (1) vs gastric bypass (4)
* Gastrectomy → malabsorption of B12 due to loss of intrinsic factor and achlorhydria. * Gastric bypass → malabsorption of B12, ADEK, iron, Ca
34
Short bowel syndrome
Malabsorption of all vitamins (esp B12, ADEK), Ca, phosphate, Mg, and K.
35
Blind loop syndrome (2)
Deficiency of B12 and ADEK
36
Corticosteroids (2)
low Vit D and Ca

Folate
39
TMP/sulfa
Folate
40
Methotrexate (2)
Folate and B12
41
Isoniazid
B6
42
PPI's (4)
B12, Mg, Ca, and iron
43
Pregnancy (2)
Increases need for folic acid and iron
44
Vegans
B12 (only found in animal products), zinc, Ca, Vit D
45
Marantic endocarditis
Fibrin and platelets deposit on heart valves due to malnutrition / wasting (seen in cancer / cachexia)
46
Kwashiorkor
Low protein in kids --> low albumin --> ascites
