- Abnormal occlusal function (occlusal disharmonies)
- Absence of neuromuscular control of mastication (parafunction BRUXISM)
- Abnormal occlusal support (PDL) Periodontitis
Give 3 examples of occlusal disharmonies.
- Premature contacts going into ICP. This can cause extreme loading in the teeth.
2.irregular contact during protrusive movements (forward sliding of the mandible jaw)
- Irregular contacts during lateral excursion side to side movements of the mandible) -
i) non-working side contacts
ii) other interferences in guidance.
Give 2 examples of parafunction.
- Bruxism and clenching - this exerts extreme pressure on the teeth
- Chewing habits leading to wear - nail biting and smoking pipes
What is periodontal disease affect on the PDL?
- Reduces surface area of PDL.
- In periodontal disease a normal masticatory force produces an abnormal occlusal load due to the lack of PDL.
- This load is increased with co-factors such as occlusal disharmonies.
- Mobility as a result increases as PDL surface area decreases
What is primary occlusal trauma?
Excessive occlusal load on periodontally healthy teeth.
It is reversible once load is sorted.
What is secondary occlusal trauma?
Excessive occlusal load on periodontally diseased teeth.
Reversible once load is sorted. However there is a lot more mobility initially as the tooth has less support.
Does occlusal trauma directly cause periodontal disease?
No, trauma doesn’t cause gingival inflammation and periodontal pockets.
Does occlusal trauma affect healing?
No
How does occlusal load affect teeth?
Increase tooth mobility
What 3 things does occlusal load not directly cause?
- Attachment loss.
- Periodontal disease.
- Periodontal breakdown
When can occulsal load cause a lot of damage alongside periodontal disease?
- If periodontitis is destructive.
- Excess occlusal laoding may enhance periodontal breakdown.
