Hodgkin’s lymphoma histology classification

A
  • Nodular sclerosing (most common): good prog.:more common in women || associated with lacunar cells
  • Mixed cellularity: good prog. || asso. large no of reed-sternberg cells
  • Lymphocyte predominat: best prog.
  • Lymphocyte depleted (rare): worst prog.

Hodgkin’s lymphoma poor prognosis factors

A
  • B sx:
    o Night sweats
    o Weight loss >10% in last 6M
    o Fever >38C
  • Other:
    o age > 45 years
    o stage IV disease
    ohaemoglobin < 10.5 g/dl
    o lymphocyte count < 600/µl or < 8%
    o male
    o albumin < 40 g/l
    o white blood count > 15,000/µl

A

RF
- HIV
- Epstein-Barr virus

PX
- Lymphadenopathy
o ~painless, non-tender, asymetrical
o Alcohol-induced lymphadenopathy pain

  • Systematic B x
    o + pruritus
  • **Other: mediastinal mass **
    o Sx i.e. cough or incidental CXR
