The company in which it is quartered.

- They are the unit’s immediate superviser and beginning of this unit’s chain of command and they are responsible for all administrative duties for those members.

All required Credentials (PAT, Drivers License, CPR and ACLS, DOH card, etc). In addition, Members shall report for duty clean, with all PPE, and in the proper uniform.

Emergency Medical Technician; Members whose job classification requires higher certifications are required to maintain those as well

DOH expires every 2 years and there is no provision for grace periods or extentions. Expiration of a card establishes the last day member may legally serve in that capacity.

Notify Immediate Supervisor 30 days prior to arrival; Member shall be afforeded 6 opportunities to complete recertification course.

EMS Training Office and Member shall receive notice at least 15 days prior to expiration.

Phone the EMS Training Office, and make a journal entry to document that the EMS Training Office and appropriate Batt. Commander were notified.

Company Officers shall maintain up to date records of the status of each member’s EMT/EMT-I/Paramedic/ACLS/CPR and at the beginning of each shift shall ensure that a minimum of one licensed EMT/CPR cardholder is on duty at all times. Company Officers shall check for the physical presence of all EMS Certifications on the first days of JAJO that their shift is scheduled to work.

MAX of 3 seperate call-ins for SL in a 12 month period

What is Sick Leave Restriction for a Single Role Provider and how long does it stay in place? When and How must a member be notified that they are at risk of being placed on sick leave restriction?
After the member has used 2 of their call-ins, they will be notified in writing that they are at risk for being put on sick leave restriction. Sick Leave restriction requires a physicians certificate or Form SF-71 and will remain in effect for a 12 month period.

When must a Single Role Provider provide a signed medical certificate or Form SF-71? What may happen if the member is unable to provide the aforementioned documentation?

All equipment and supplies. Deficiencies shall be noted in teh journal, to the platoon commander, and on the appropriate 54s. 54s shall be completed at the beginning of the shift.

Meds due to expire shall be evenly exhcanged with newer medication 30 days prior to expiration. Items that do not have a day are considered to expire the first day of the month.

On-duty company commander and Batt. Superviser shall be notified IMMEDIATELY

Medication (Rx) Due to Expire Checklist and it shall be completed on the first day of the month and immediately reviewed by the on-duty company commander.

Who will pick up expired medications and how often? Whose resonsibility is it to package the expired medicaitons properly?
The Logistics Van Driver shall pick up expired meds on a monthly basis. Proper packaging shall be the responsibility of the EMS provider doing daily inventories.

When shall refueling of EMS Vehicles Occur?
At or below 1/2 tank and preferably during off-peak hours (0100-0900 hours)

Unit will be kept clean and neat at all times. Exterior washed daily; interior periodically as needed.
