To reinstate a lapsed pharmacy license, the pharmacist must:
1.) Pay the license fees and delinquency fees for all years during the time the license was lapsed
2.) Provide certification of completion for continuing education requirements for all years in which the license was lapsed
3.) If the license was lapsed > 1 year, pass the MPJE with a score of ≥ 75
4.) Complete an application for licensure, with a fingerprint card or other documentation to conduct a criminal background check

Meet minimal standards of training and experience in the handling of radioactive materials
Submit to the Board evidence of current certification in nuclear pharmacy by the Board of Pharmaceutical Specialties, or evidence that they meet both the following:
Certification of a minimum of six-month on-the-job training under the supervision of a qualified nuclear pharmacist in a nuclear pharmacy providing radiopharmaceutical services; and
Certification of completion of a nuclear pharmacy training program in a college of pharmacy or a nuclear pharmacy training program approved by the Board.
Receive a letter of notification from the Board that the evidence submitted by the pharmacist meets the above requirements and has been accepted by the Board.
