Dermatomyositis

A

Heliotrope Rash: periorbital violaceous erythema, mask over nasal bridge
Gottron Papules: pathognomonic flat violaceous papules on IP/MCP joints
Violaceous, s/t scaly rash over extensor surface of elbows/knees
Muscles: symmetric proximal weakness, s/t tenderness
*always consider cardiac muscle involvement
Labs: CPK, ESR s/t wnl initially
ANA + w/speckled pattern
lymphopenia, changes in WBC differential
MRI: very sensitive for dz in muscle
Tx: prednisone, MTX
