Difference between adverse event and sentinel event:
both do harm to patient but sentinel is unexcpected and should never be allowed to happen
Difference between incident and accident
incident had effect on context but accident has effect on the state of health
When is disclosure mandatory
when there is an accident (effect on state of health) with consequences
ex: a patient got scratched, he needs stitches.
