What labs should you order on pt w/ thyroid prob? What imaging?
TSH, FT4, CBC & CMP, anti-TGB antibodies, TPO (thyroperoxidase) antibodies
Ultrasound, RAI uptake (which isn’t useful in Hashimoto’s)
What are cold and hot nodules?
Cold nodules = more likely CA
Hot nodules rarely CA
What n can be damaged in thyroid resection surgery?
Laryngeal n
How should one initiate Levothyroxine tx?
Slowly, no matter the TSH, different people respond differently! So low doses might work for some.. others might need more.. don’t want to overshoot
Take in the AM (w/o food) and drink full glass of water!
What test is ordered to rule out pheo?
Serum metanephrines (epi.. NE..)
What lab findings are seen in DeQuervains? Physical findings?
Elevated CRP (C-reactive protein)
Tenderness of thyroid to palpation, mild fever
What lab results are associated w/ Hypoparathyroidism?
Low: Ca, urinary Ca, PTH
High: serum phosphates
Normal: alkaline phosphate
What’s a likelihood in someone w/ recurrent kidney stones made of calcium?
Hyperparathyroidism
What are the lab results of someone w/ hyperparathyroidism?
Low: phosphate
High: PTH, calcium, vit D
What test is used for pts w/ expected osteoperosis? What’s the tx for osteoperosis?
DEXA-scan, you get a T
Bisphosphanates
What’s Paget’s dx?
State where a pt’s bones are so fragile that modest bumps can break a bone
Osteomalacia is what? What lab values are seen?
Painful proximal muscle weakness (low ATP materials) and bone tenderness/weakness often found in the pelvic girdle d/t vit D def (seen as a result of env changes, humans aren’t exposed to sunlight as much), Ca def, or phosphate def
Low: bone density, 25-hydroxy-vitD, Ca, urine Ca, phosphate in urine
High: PTH, alk phos
What dx is osteomalacia in kids?
Rickets
What’s the tx for osteomalacia and Rickets?
Ca supplementation (elderly don’t absorb it as well from dietary sources) aka Calcitriol
What are lab findings in osteoperosis?
Everything is likely normal aside from Vit D, a DXA scan would also show dec bone density
What are the T score categories?
T score > -1 = normal
T score -1 to -2.5 = osteopenia (low bone density)
T score < -2.5 = osteoperosis
T score < -2.5 + fracture = severe osteoperosis
