What is pathologic hyperbilirubinemia?

· Appears BEFORE 24 hours of age
· Results of underlying disease-SERIOUS!!
· Infection
· Blood incompatibility
· Abnormal liver

· Birth Trauma
· Preterm status
· Poor feeder
· Polycythemia

What is an example of ABO incompatibility? What is an Rh incompatibility?
*Mom is type 0 and baby is not
*Mom is Rh negative and Baby is positive
What is phototherapy?
Baby placed under UV light to break down bilirubin in skin
What are the nursing interventions/care for infant phototherapy?
Nursing Interventions/Care:
o Skin assessments/skin care/labs to watch bilirubin levels
o Feed often and track I&O
o Monitor infant behavior
o Support thermoregulation
o Cover eyes and genitals while under lights
o Support bonding
o Support and educate parents-transfusion a possibility.
What are afterpains?
When women experience discomfort and cramping due to the uterus contracting back to normal size pre-pregnancy
Who has worse afterpains?
- overstretched uterus
- polyhydraminos (diabetes)
- During breastfeeding
Afterpains last how long typically?
shouldn’t last more than 48 hours
What are some suggested comfort measures for afterpains?
heat on belly, NSAIDS
What are lochia warning and danger signs?
Lochia warning and danger signs
- temp above 100.4 F, severe pain, foul smell, bleeding getting heavier
How often should a mother breastfeed?
every 2 hours (8-12 times in 24 hours; when needed)
*about the same for formula feeding
What are some success tips for breastfeeding?
- try difference positions
- start on the opposite breast each time you feed
What does “let-down” mean?
Let down - release of milk
*Heat encourages let down (not cold)
How soon is breastfeeding encouraged after birth?
within an hour
How can a mother tell if a baby is getting adequate amount of milk during breastfeeding?
- not showing hunger cues
- baby falls asleep
- baby gains weight
What should mothers know about burping?
*All babies (breastfed or bottlefed) need burped … between breasts and after breast!
What are some warning signs with breastfeeding?
*s/s mastitis (treatment 10 days of antibiotics and bedrest)
*cracked/bleeding breasts
What are ways to encourage milk drying/comfort?
- avoid stimulation to the nipple
- medications (decongestants) and herbs (or cabbage)

DOs:
- keep incisions clean

- no redness, swelling, purulent drainage, no wound opening
How long should mothers use the squirt bottle after C-section or vag delivery?
Until theres no more lochia
What are some things one should know about baby bath’s?
- Frequency for baths- 2-3x a week
- Safety tips & water temperature- 100F
- Bathing process- clean from cleanest to dirtiest
