Pathology- Endocrine Pancreas Flashcards
(38 cards)
What is a characteristic of nondiabetic newborns of diabetic mothers?
Increase in the number and size of islets
What happens to fetal islets secondary to meternal hyperglycemia?
They undergo hyperplasia
What are prime targets of diabetes?
Kidneys
What is second only to MU as cause of death in diabetes?
Renal failure
What are 3 lesions seen in kidneys with diabetes?
- Glomerular lesions
- Renal vascular lesions
- Pyelonephritis (including necrotizing papilitis)
What is nodular glomerulosclerosis also called?
- Intercapillary glomerulusclerosis
2. Kimmelstiel-Wilson disease
What is seen in nodular glomerulosclerosis?
Ovoid or spherical, laminated, nodules of matrix
Where are nodules in the glomerulus in nodular glomerulosclerosis?
Peripephery
True or False: Nodules in nodular glomerulosclerosis are PAS positive?
TRUE
What are nodules surrounded by in nodular glomerulosclerosis?
Patent peripheral capillary loops or loops that are markedly dilated
What is seen in advanced nodular glomerulosclerosis?
Enlarged nodules
What are 3 consequences of glomerular and arteriolar lesions?
- Kidney suffers from ischemia
- Develops tubular atrophy
- Interstitial fibrosis
What does the picture of nodular glomerulosclerosis look like?
Petals on a flower
What is an insulinoma?
B-cell tumor
True or False: Insulinoma is the most common of pancreatic endocrine neoplasms?
TRUE
What happens in an insulinoma that induces clinically significant hypoglycemia?
Elaboration of sufficient insulin
What is the clinical picture of an insulinoma?
Hypoglycemia episodes: Blood glucose under 50, CNS manifestations (confusion, stupor, loss of consciousness), precipitated by fasting or exercise (promptly relieved by feeding/parenteral administration of glucose)
Describe some characteristics of insulinoma
Benign, solitary, small (under 2 cm), encapsulated, pale to red-brown nodules
What % of cases of insulinoma are carcinomas?
10%… local invasion and distant mets
What is seen microscopically in an insulinoma?
Giant islets with preservation of regulat cords and preservation of orientation to vasculature
What is deposited in extracellular tissue in an insulinoma?
Amyloid
What labs are seen with insulinoma?
High circulating levels of insulin
High insulin-to-glucose ratio
What is treatment for an insulinoma?
Surgical removal of tumor… followed by prompt reversal of the hypoglycemia
What is in the Ddx of insulinoma?
- Abnormal insulin sensitivity
- Diffuse liver disease
- Inherited glycogenoses
- Ectopic production of insulin (retroperitoneal fibromas and fibrosarcomas)
- Self-injection of insulin
